Form Builder is a Shopify app that lets you create custom forms for your online store — without writing a single line of code. Once your form is ready, publish it directly to any page in your store. Submissions are collected automatically, and you can view them right inside your Shopify admin, get notified by email, and even send data to tools like Google Sheets, Mailchimp, and Slack.

#How it works

Form Builder follows a simple four-step workflow:

#1. Create your form

Start from scratch, pick one of 31 templates, or describe what you need and let the AI build it for you. You will land in the drag-and-drop form builder where you can add any fields you need.

#2. Customize the look

Use the Design tab to choose a layout type, input style, fonts, and colors that match your store's branding. Set up what happens after a customer submits — show a thank-you message, redirect to another page, or display a discount code.

#3. Publish to your store

Pick which page the form should appear on — your homepage, a specific page, or a product page. Choose whether to embed it inline, show it as a floating button, or display it as a popup.

#4. Collect and manage submissions

Every time a customer fills out your form, the submission is saved automatically. View responses in your dashboard, get email notifications, export data to CSV or Excel, and send submissions to your favorite tools through integrations.
